In order to be able to participate in high school athletics each athlete must be cleared by the CVC Athletic Department. There are two things that need to be completed each year in order to be elegible to participate.

1. Student Central - Online Emergency Medical Packet and all CIF/School Forms
   • Student Central Parent/Student Guide
2. Physical Form
   • Physical forms must be signed and assigned a date by a physician in order to be valid
   • All physicals expire 1 year from the date on the physical form.
